The 4 Zones of Gynecomastia
To achieve the best aesthetic outcome, it is crucial to identify not just the type of gynecomastia, but also the location of the tissue. Dr. Vali breaks down the chest into 4 distinct zones:
- 1Zone 1: The Nipple-Areolar Complex.
This is where the glandular tissue typically resides.
- 2Zone 2: The Breast Mound.
The area immediately surrounding the areola.
- 3Zone 3: The Lateral Chest.
The side of the chest, often containing excess fat rolls.
- 4Zone 4: The Upper Chest.

The 4 Types of Gynecomastia
Dr. Vali classifies gynecomastia into four main types based on the severity, tissue composition, and skin elasticity.

Type 1: Puffy Nipple
This is the most common form of gynecomastia. It is characterized by a localized button of glandular tissue concentrated directly underneath the nipple-areolar complex. The surrounding chest is usually flat or muscular, but the nipple projects outward, creating a 'puffy' appearance.

- Localized gland under the nipple
- Puffy or protruding nipple
- Common in bodybuilders and fit individuals
Recommended Treatment:
Gland excision is the primary treatment. VASER liposuction is often used to feather the edges and contour the surrounding area.

Type 2: Mild Gynecomastia
Type 2 involves a small to moderate amount of excess breast tissue that extends beyond the boundaries of the areola. The chest appears slightly fuller, but the skin is tight and there is no significant sagging.

What is Pseudo-Gynecomastia?
Not all chest enlargement is true gynecomastia. Pseudo-gynecomastia refers to the accumulation of excess fat in the chest area without the presence of enlarged glandular tissue. This condition is often associated with weight gain or obesity.
How it differs:
- Composed primarily of adipose (fat) tissue
- No hard lump under the nipple
- Can often be improved with diet and exercise
Treatment:
Since there is no gland to excise, the primary treatment is VASER Liposuction to remove the stubborn fat and contour the chest.
